Gift Card Terms

A stored-value gift card with a face value of $1,788 USD, redeemable toward eligible products and services offered by CoreAge Rx. It is not a credit, debit or charge card, and it carries no interest.

The card is issued to the single verified winner of this giveaway, and only that person may redeem it. It is awarded as the prize in the promotion described in the Official Rules, and it is not sold, issued or made available to anyone else. Entering the giveaway does not create any entitlement to a card unless you are selected and verified as the winner.

The card does not expire and no dormancy, inactivity or service fee is charged against its balance at any time.

The card is not transferable. It is issued to the verified winner and may be redeemed only by that person. It may not be sold, bartered, auctioned or assigned to another person. A card offered for resale is void.

The card may be combined with other discounts and promotional offers unless the terms of the specific offer say otherwise. Individual promotions may exclude gift card payment, and where they do, that exclusion applies.

The card may be applied to subscription orders where CoreAge Rx supports it at the time of purchase.

Applying the card to a subscription does not cancel the subscription or stop future billing. Once the card balance is exhausted, any recurring charge continues against the payment method on file at the then-current price. Cancel the subscription directly with CoreAge Rx if you do not want it to continue.

Subject to CoreAge Rx's terms at the time of purchase, the card may be applied to:

• Non-prescription products, including supplements and topical products
• Consultation and intake fees
• Membership or programme fees, where charged
• Prescription products, subject to the restrictions below
• Shipping charges
• Applicable sales tax

Where CoreAge Rx does not permit gift card payment for a particular charge, that restriction applies and the balance remains available for other eligible purchases.

A prescription product may be purchased with the card only after the intake process is completed and a licensed healthcare provider has evaluated the purchaser and approved treatment.

Holding this card does not make anyone eligible for a prescription medication. Eligibility and every treatment decision rest solely with the licensed healthcare provider. The card cannot override, influence or substitute for that decision, no particular medication is promised, and no medication is guaranteed to be available. Compounded medications are not FDA-approved products.

If the provider does not approve treatment, the balance remains on the card and may be applied to other eligible products and services.

Any unused balance stays on the card and may be applied to a later eligible purchase. The card is not reloadable.

The card is not refundable and its value is not returned as cash, except where required by applicable law. Where a purchase paid for with the card is refunded, the refund is returned to the card balance rather than as cash.

The card is not redeemable for cash, except where applicable state law requires cash redemption — several US states require a cash refund of a remaining balance below a stated threshold on request. Where such a law applies to the winner, it is honoured.

The card may be redeemed only for products and services CoreAge Rx is able to supply to the winner's location. Product availability differs by state, and prescription products in particular are subject to the states in which CoreAge Rxclinicians are licensed. A card cannot be redeemed for a product that is not lawfully available where the winner lives.

Treat the card like cash. Report a lost or stolen card to CoreAge Rx immediately.

A card may be replaced at CoreAge Rx's discretion where the winner can be identified and the remaining balance verified from CoreAge Rx's records. Neither CoreAge Rx nor the Sponsor is responsible for a balance spent before a loss is reported.

The card may not be:

• Resold, auctioned, bartered or transferred to another person
• Used to purchase another gift card or stored-value instrument
• Used for any unlawful purpose
• Used after being reported lost, stolen or void
• Used to obtain a prescription product without completing intake and receiving provider approval
• Used in a way that misrepresents who is receiving the product or service

Misuse voids the remaining balance without refund, to the extent permitted by law.
